What to bring for your office visit:

• All previous imaging you may have for the body part in question (Example: X-rays, MRIs, CTs) - Please bring the hard films or CDs with a copy of the report if available.

• Insurance Card

• Photo ID

If your injury was caused by an Auto Accident:

• Name of auto insurance company

• Date of accident

• Billing address of auto insurance company

• Claim number

• Name of adjuster associated with this claim

• Phone number of adjuster

• Copy of auto insurance card

• If you have health insurance, please bring your health insurance card 

If the patient is a minor:

• If a parent or guardian is unable to bring the patient for their visit, please provide the office with a letter of consent, allowing someone else to authorize treatment.
